Mixed Results Seen With 'Off-Label' Use of Antipsychotics (HealthDay)

TUESDAY, Sept. 27 (HealthDay News) -- Even though off-label use of atypical antipsychotic drugs is common, a new review finds that such use is only effective in a few conditions and can be harmful in others.

Also known as "second-generation" antipsychotics, these drugs are approved in the United States for treatment of schizophrenia, bipolar disorder and depression. But off-label use -- or use not approved by the U.S. Food and Drug Administration -- of these drugs to treat a number of other conditions is growing.

Researchers analyzed findings from 393 clinical studies to gauge effectiveness and uncover harmful side effects when Risperdal, Zyprexa, Seroquel, Abilify, Geodon, Saphris, Fanapt and Invega were used outside of FDA-approved indications.

Off-label use included treatment for behavioral symptoms in dementia, anxiety, depression, obsessive-compulsive disorder, eating disorders, post-traumatic stress disorder, insomnia, personality disorders and substance abuse.

The investigators found that Abilify (aripiprazole), Zyprexa (olanzapine) and Risperdal (risperidone) were associated with small but statistically significant benefits for elderly people with dementia symptoms, such as aggression, psychosis and mood changes.

Seroquel (quetiapine) was 26 percent more effective than placebo in people with generalized anxiety disorder, and Risperdal was four times more effective than placebo in people with obsessive-compulsive disorder.

There was no evidence that atypical antipsychotics benefited people with substance abuse or eating disorders, according to Dr. Alicia Ruelaz Maher, of RAND Health in Santa Monica, Calif., and colleagues.

The researchers also found that certain atypical antipsychotics increased the risk of death, stroke, movement disorders and urinary tract symptoms in elderly patients. In younger patients, some of the drugs caused problems such as weight gain, fatigue, sedation, and akathisia, also known as restless leg syndrome.

The study is published in the Sept. 28 issue of the Journal of the American Medical Association.

"The benefits and harms vary among atypical antipsychotic medications for off-label use," the researchers concluded in a journal news release. "This evidence should prove useful for clinicians considering off-label prescribing of atypical antipsychotic medications, and should contribute to optimal treatment decision making for individual patients with specific clinical symptoms and unique risk profiles."

Obama pitches jobs plan in Denver (AP)

LOS ANGELES ? President Barack Obama is heading to Denver to pitch his jobs plan in the city where he accepted the Democratic nomination. But as he's acknowledged at fundraisers up and down the West Coast, this election isn't like 2008.

Obama told a star-studded Hollywood fundraiser Monday night that this time around people are weary, and the energy of 2008 ? when he was an exciting new face ? is going to have to be generated in a different way.

Obama has held seven fundraisers over two days in Washington state and California in advance of an important fundraising deadline. Tuesday in Denver, he will visit a high school to speak about the $450 billion jobs plan, mixing tax credits and public works spending, that he wants Congress to pass.

The president is expected to focus Tuesday on a proposal in the bill that would allocate $25 billion in new spending to repair and modernize public schools. The White House said Colorado's share of the funding could total $265 million and would support up to 3,400 jobs.

With the economy stuck and unemployment high as the 2012 campaign heats up, Obama has been reminding supporters that he never told them governing would be easy.

His own chief political adviser, David Axelrod, underscored that reality Tuesday, when he cast the president's re-election bid as "a Titanic struggle."

With the tough political landscape in mind, Obama has used his West Coast swing to get in some shots at the GOP presidential field.

"I urge all of you to watch some of these Republican debates. It's a different vision about who we are, who we stand for," Obama told about 120 Hollywood supporters at Los Angeles' Fig & Olive restaurant. Guests paid $17,900 each to attend.

The American people want a "bold and generous" vision of America, Obama said, not the "cramped" one he said the Republicans are offering. "That's what this election is about. It's about values, it's about character, it's about who we are."

Republicans have panned Obama's West Coast swing, criticizing the president for being more interested in saving his own job than creating new jobs for Americans. And Republican National Committee spokeswoman Kirsten Kukowski said Obama was "treating the West Coast as an ATM for his campaign coffers as he makes his way toward the first billion-dollar campaign."

UAW shifts talks from Chrysler to Ford

The United Auto Workers union shifted the focus of its contract talks to Ford Wednesday night after failing to reach a deal with Chrysler.

The move could mean trouble in the talks with Chrysler, which was close enough to a deal with the union that CEO Sergio Marchionne flew from Europe to the U.S. late Tuesday in case his signature was needed.

But late Wednesday, the union said in a short statement that its attention would now shift to Ford, where little progress has been reported. The UAW gave no reason for the change.

"They obviously hit a pothole of some sort, and they took an alternate route by going to Ford," said Harley Shaiken, a professor at the University of California at Berkeley who specializes in labor issues.

Earlier Wednesday, the union and Chrysler announced a second contract extension so they could continue negotiations, this one until Oct. 19. But the talks stopped Wednesday evening, and it was unclear when they would restart.

Shaiken said changes from one company to another during the talks are unusual but have occurred in previous negotiations. He's been following UAW labor talks for four decades.

Neither Chrysler nor the union would comment about the lengthy contract extension.

Ford spokeswoman Marcey Evans said Wednesday night that the company looks forward to negotiating a deal that's fair to employees and "allows Ford to become more competitive."

Ford has the highest hourly labor costs in the industry and likely will try to find ways to cut them to match competitors.

The UAW reached an agreement with General Motors late Friday. GM workers are expected to begin voting on that deal soon. It includes a $5,000 bonus if the contract is ratified and a new profit-sharing plan in place of annual raises.

But Chrysler, which is much smaller, may not have the resources to match the bonuses and profit-sharing that GM offered its workers last week. The automaker's main objective is to hold labor costs steady at $49 per hour, among the lowest in the U.S. auto industry.

Chrysler workers know that they may not get as good of a deal as GM workers, Kristin Dziczek, head of the labor and industry group at the Ann Arbor, Mich.-based Center for Automotive Research, told the Automotive Press Association in Detroit Wednesday.

Chrysler reported a $254 million net loss during the first half of the year, while GM earned $5.4 billion. Chrysler workers also know the company still needs money to retool plants and revamp more of its models, Dziczek said. Chrysler is upgrading its lineup with technology from Fiat, which owns a majority of Chrysler.

"I think the UAW gets that, and they understand what they're going to have to do to get an agreement at Chrysler," Dzicek said.

But she also said Marchionne, who also runs Fiat, has been tough on Italian unions.

The talks with Chrysler already have hit some bumps. In a letter sent to UAW President Bob King last week, Marchionne accused King of failing to show up to complete negotiations as scheduled. But Marchionne said Tuesday that he was optimistic they'd reach an agreement soon.

King wouldn't discuss the letter, but said Tuesday that he has a good relationship with Marchionne.

Complicating the Chrysler talks is the UAW's indirect ownership of company stock. More than 40 percent of Chrysler is owned by a UAW trust fund that pays retiree health care bills. That puts the union in a difficult position: It must win more money or job guarantees from Chrysler without hurting the company's ability to turn a profit and boost the stock price when Chrysler eventually goes public again.

Meanwhile, negotiations at Ford Motor Co. had slowed while the union concentrated on the other two companies. Ford may have to offer a sweeter deal than GM because it has made more money and because workers are upset with the company about restoring white-collar merit pay when union workers got no raises.

GM's deal promises news jobs and car production in the U.S, a top union concern. It also offers buyouts for more expensive, longtime union workers.

Under the deal, GM's union workers get a $5,000 signing bonus and profit-sharing checks that will likely exceed the $4,300 workers received this year. Longtime workers won't get a pay raise, helping GM contain costs. But entry-level workers will get raises of up to 24 percent during the contract's four years.

JACKSON, Ga. (AP) ? Georgia executed Troy Davis on Wednesday night for the murder of an off-duty police officer, a crime he denied committing right to the end as supporters around the world mourned and declared that an innocent man was put to death.

As he lay strapped to a gurney in the death chamber, the 42-year-old told relatives of Mark MacPhail that he was not responsible for his 1989 slaying. "I did not have a gun," he insisted.

"All I can ask ... is that you look deeper into this case so that you really can finally see the truth," he said.

He asked his friends and family to "continue to fight this fight." Of prison officials he said, "may God have mercy on your souls. May God bless your souls."

Davis was declared dead at 11:08 p.m. The lethal injection began about 15 minutes earlier, after the Supreme Court rejected an 11th-hour request for a stay.

"Justice has been served for Officer Mark MacPhail and his family," state Attorney General Sam Olens said in a statement.

The high court did not comment on its order, which came about four hours after it received the request and more than three hours after the planned execution time.

Hundreds of thousands of people signed petitions on Davis' behalf, and prominent supporters included an ex-president and an ex-FBI director, liberals and conservatives. His attorneys said seven of nine key witnesses against him disputed all or parts of their testimony, but state and federal judges repeatedly ruled against him ? three times on Wednesday alone.

MacPhail's widow, Joan MacPhail-Harris, said there was "nothing to rejoice," but that it was "a time for healing for all families."

"I will grieve for the Davis family because now they're going to understand our pain and our hurt," she said in a telephone interview from Jackson. "My prayers go out to them. I have been praying for them all these years. And I pray there will be some peace along the way for them."

Davis' supporters staged vigils in the U.S. and Europe, declaring "I am Troy Davis" on signs, T-shirts and the Internet. Some tried increasingly frenzied measures, urging prison workers to stay home and even posting a judge's phone number online, hoping people will press him to put a stop to the lethal injection. President Barack Obama deflected calls for him to get involved.

"They say death row; we say hell no!" protesters shouted outside the Jackson prison before Davis was executed. In Washington, a crowd outside the Supreme Court yelled the same chant.

As many as 700 demonstrators gathered outside the prison as a few dozen riot police stood watch, but the crowd thinned as the night wore on and the outcome became clear. The scene turned eerily quiet as word of the high court's decision spread, with demonstrators hugging, crying, praying, holding candles and gathering around Davis' family.

Laura Moye of Amnesty International said the execution was "the best argument for abolishing the death penalty."

"The state of Georgia is about to demonstrate why government can't be trusted with the power over life and death," she said before Davis was put to death.

About 10 counterdemonstrators also were outside the prison, showing support for the death penalty and MacPhail's family.

Members of Davis' family who witnessed the execution left without talking to reporters. MacPhail's son and brother also attended.

"I'm kind of numb. I can't believe that it's really happened," MacPhail's mother, Anneliese MacPhail, said in a telephone interview from her home in Columbus, Ga. "All the feelings of relief and peace I've been waiting for all these years, they will come later. I certainly do want some peace."

Of Davis' claims of innocence, she said, "He's been telling himself that for 22 years. You know how it is, he can talk himself into anything."

Davis' execution had been stopped three times since 2007, but on Wednesday he ran out of legal options. The pardons board rejected him, and Georgia's governor does not have the power to grant condemned inmates clemency.

As his last hours ticked away, an upbeat and prayerful Davis turned down an offer for a special last meal as he met with friends, family and supporters.

"Troy Davis has impacted the world," his sister Martina Correia said at a news conference. "They say, 'I am Troy Davis,' in languages he can't speak."

His attorney Stephen Marsh said Davis would have spent part of Wednesday taking a polygraph test if pardons officials had taken his offer seriously.

"He doesn't want to spend three hours away from his family on what could be the last day of his life if it won't make any difference," Marsh said.

Davis' supporters include former President Jimmy Carter, Pope Benedict XVI, a former FBI director, the NAACP, several conservative figures and many celebrities, including hip-hop star Sean "P. Diddy" Combs.

"I'm trying to bring the word to the young people: There is too much doubt," rapper Big Boi, of the Atlanta-based group Outkast, said at a church near the prison.

At a Paris rally, many of the roughly 150 demonstrators carried signs emblazoned with Davis' face. "Everyone who looks a little bit at the case knows that there is too much doubt to execute him," Nicolas Krameyer of Amnesty International said at the protest.

The U.S. Supreme Court gave Davis an unusual opportunity to prove his innocence in a lower court last year, though the high court itself did not hear the merits of the case.

He was convicted in 1991 of killing MacPhail, who was working as a security guard at the time. MacPhail rushed to the aid of a homeless man who prosecutors said Davis was bashing with a handgun after asking him for a beer. Prosecutors said Davis had a smirk on his face as he shot the officer to death in a Burger King parking lot in Savannah.

No gun was ever found, but prosecutors say shell casings were linked to an earlier shooting for which Davis was convicted.

Witnesses placed Davis at the crime scene and identified him as the shooter, but several of them have recanted their accounts and some jurors have said they've changed their minds about his guilt. Others have claimed a man who was with Davis that night has told people he actually shot the officer.

"Such incredibly flawed eyewitness testimony should never be the basis for an execution," Marsh said. "To execute someone under these circumstances would be unconscionable."

State and federal courts, however, have repeatedly upheld Davis' conviction. One federal judge dismissed the evidence advanced by Davis' lawyers as "largely smoke and mirrors."

"He has had ample time to prove his innocence," MacPhail-Harris said. "And he is not innocent."

The last motion filed by Davis' attorneys in Butts County Court challenged testimony from two witnesses and disputed testimony from the expert who linked the shell casings to the earlier shooting involving Davis. Superior Court Judge Thomas Wilson and the Georgia Supreme Court rejected the appeal, and prosecutors said the filing was just a delay tactic.

The National Association for the Advancement of Colored People, which helped lead the charge to stop the execution, said it considered asking Obama to intervene, even though he cannot grant Davis clemency for a state conviction.

Press secretary Jay Carney issued a statement saying that although Obama "has worked to ensure accuracy and fairness in the criminal justice system," it was not appropriate for him "to weigh in on specific cases like this one, which is a state prosecution."

Dozens of protesters outside the White House called on the president to step in, and about 12 were arrested for disobeying police orders.

Davis was not the only U.S. inmate put to death Wednesday evening. In Texas, white supremacist gang member Lawrence Russell Brewer was put to death for the 1998 dragging death of a black man, James Byrd Jr., one of the most notorious hate crime murders in recent U.S. history.

Davis' best chance may have come last year, in a hearing ordered by the U.S. Supreme Court. It was the first time in 50 years that justices had considered a request to grant a new trial for a death row inmate.

The high court set a tough standard for Davis to exonerate himself, ruling that his attorneys must "clearly establish" Davis' innocence ? a higher bar to meet than prosecutors having to prove guilt. After the hearing judge ruled in prosecutors' favor, the justices didn't take up the case.

New $322M Augusta hospital aims to retain patients, attract ...

AUGUSTA, Maine ? On the outskirts of Augusta on Friday, officials from the MaineGeneral Health system will break ground on a brand new $322 million hospital. The 192-bed facility is scheduled to open in 2014 and will replace three smaller, aging MaineGeneral hospitals in the region ? the Thayer and Seton facilities in Waterville and the former Augusta General Hospital in Augusta. The new hospital will feature all private inpatient rooms,10 operating rooms, and the latest in imaging, diagnostic and therapeutic technologies.

The MaineGeneral project is funded through a $35 million donation from the Harold Alfond Foundation and more than $280 million in 30-year bonds sold through the Maine Health and Higher Educational Facilities Authority. It won the approval of the state?s Certificate of Need office in November 2010. It is expected to open in June of 2014.

The objective in building the new hospital, according to MaineGeneral President and CEO Scott Bullock, is to attract health professionals and better serve the health care needs of the region ? not to draw patients away from larger medical centers in Bangor, Lewiston and Portland.

?This project will help us compete,? Bullock said in a phone interview on Wednesday, ?but we?re not doing this to draw new patients to Augusta.? Rather, he said, the new facility is needed to attract skilled medical providers ? physicians, nurses and others ? to central Maine, making it easier for existing patients to get the care they need without leaving the area.

?Physicians prefer to have their team members together and to practice in a new, state-of-the-art facility,? Bullock said. Consolidating inpatient care to one hospital also will allow health care providers to be on call less often, he said, reducing stress and enhancing their quality of life.

MaineGeneral Health is the third largest health care system in the state, after MaineHealth in Portland and Eastern Maine Healthcare Systems in Brewer. According to a recent report from the Maine Development Foundation, MaineGeneral has revenues of over $320 million and an annual payroll of about $138 million for over 3,500 employees.

In addition to the three small 1960s-era hospitals that will be replaced by the new facility, the

system includes more than 60 outpatient offices and other services in nine different communities. The latest addition is the Harold Alfond Center for Cancer Care, which opened in 2007 on the same 160-acre site that will be home to the new hospital.

The Maine Development Foundation report found that in 2007, the number of hospital patients seeking care at more modern and better-equipped facilities outside the MaineGeneral service region was equal to 30 percent of the organization?s total share of patient visits that year, representing a loss of over $64 million in community hospital revenues.

The outdated facilities also affect the ability to attract and retain high-caliber medical care providers, according to the report. Currently, the MaineGeneral Health hospitals have a 20 percent physician vacancy rate, anticipated to grow to 38 percent if conditions do not change.

Bullock said that while residents of Waterville and Augusta are used to having inpatient care available right in their communities, the new site is centrally located and easy to get to. Getting to the new hospital, and the cancer care center, will be made easier by the opening of a new access road off Exit 113 on I-95 sometime before the targeted completion of the facility in June 2014, he said. That road, Bullock said, will not only serve the MaineGeneral site but also open access to other relatively undeveloped areas to the west of the Interstate.

At Eastern Maine Healthcare Systems in Brewer, President and CEO Michelle Hood said the MaineHealth project poses little threat to her system?s operations.

Eastern Maine Medical Center, the system?s largest hospital, is one of just two hospitals in the state ? the other one is Maine Medical Center in Portland ? offering high-level, comprehensive services such as neonatal intensive care, invasive cardiac procedures and advanced orthopedic surgeries. EMMC, along with Maine Medical Center and Central Maine Medical Center in Lewiston, also is a designated Level One Trauma Center, prepared to respond to the most severe emergencies. These services will not be available at MaineGeneral?s new hospital, Hood noted, and EMMC will continue to serve the region of Maine from Augusta north.

The 25-bed Inland Hospital in Waterville, the southernmnost affiliate of Brewer-based Eastern Maine Healthcare Systems, will stay open, Hood said.

Hood said long-standing plans to improve facilities at EMMC?s State Street campus are moving forward incrementally. While new surgical suites and a planned tower of private patient rooms may be delayed, other changes are already under way, including revisions to the hospital?s emergency entrance and the utilities infrastructure.

?We are very interested in proceeding with this project, but we want to do it at the right time,? Hood said. The protracted economic downturn, projected changes to Medicare and Medicaid reimbursements, and a new national focus on outpatient care is pressuring all hospitals to reassess their priorities, Hood said.

Bullock said Maine?s health care sector must respond to consumer demand. The state?s aging population, and the pervasive influence of the baby boom generation, will place more pressure on hospitals and other providers in coming years.

?We?re all just going to have to be there to take care of people,? he said. ?The demographics are the primary determinant of the health care economy.?

Bullock said MaineGeneral Health?s Thayer facility will be converted for outpatient services, including renal dialysis, physical and occupational therapies and a full-service emergency department. The Seton and Augusta General campuses will be closed.

Typhoon buffets Tokyo, dumps rain in tsunami zone (AP)

TOKYO ? A powerful typhoon slammed into Japan Wednesday, halting trains and leaving 13 people dead or missing in south-central regions before grazing a crippled nuclear plant and heaping rain on the tsunami-ravaged northeast.

Officials at the Fukushima Dai-ichi plant, where engineers are still struggling with small radiation leaks due to tsunami damage, expressed relief that Typhoon Roke's driving winds and rain caused no immediate problems there other than a broken security camera.

"The worst seems to be over," said Takeo Iwamoto, spokesman for plant operator Tokyo Electric Power Co., after the storm passed just west of the plant on its way north.

But the typhoon brought new misery to the northeastern region already slammed by the March 11 earthquake and tsunami, dumping up to 17 inches (42 centimeters) of rain in some areas.

Authorities warned of a high risk of mudslides in that region. Hundreds of tsunami survivors in government shelters in the Miyagi state town of Onagawa were forced to evacuate for fear of flooding.

More than 200,000 households in central Japan were without electricity late Wednesday. Police and local media reported 13 people dead or missing in southern and central regions, many of them believed swept away by rivers swollen with rains.

The storm, packing sustained winds of up to 100 mph (162 kph), made landfall in the early afternoon near the city of Hamamatsu, about 125 miles (200 kilometers) west of Tokyo. The fast-moving storm went past the capital in the evening and then headed up into the northeast, where it was losing strength.

In Tokyo, where many rush hour commuter trains were suspended, thousands of commuters trying to rush home were stuck at stations across the sprawling city.

"The hotels in the vicinity are all booked up, so I'm waiting for the bullet train to restart," Hiromu Harada, a 60-year-old businessman, said dejectedly at Tokyo Station.

Fire department officials reported three people injured in Tokyo. In the trendy shopping district of Shibuya, winds knocked a tree onto a sidewalk, but no one was hurt. Pedestrians struggled to walk straight in powerful winds that made umbrellas useless.

At the Fukushima plant, engineers are still working to stabilize the reactors six months after three of them melted down when the tsunami disabled the plant's power and back-up generators.

Iwamoto said the storm passed without damaging the reactors' cooling systems, which are crucial to keeping them under control. However, a closed-circuit camera that shows exteriors of the reactor buildings abruptly stopped, he said.

Workers were trying to prevent pools of contaminated water from flooding and leaking outside the complex, said Junichi Matsumoto, another power company spokesman.

"The contaminated water levels have been rising, and we are watching the situation very closely to make sure it stays there," Matsumoto told reporters.

As the storm headed further into the north, it triggered landslides in parts of Miyagi state that already were hit by the March disasters. The local government requested the help of defense troops. Dozens of schools canceled classes.

The disaster-struck region had a chilling reminder of its earlier disasters when a magnitude-5.3 earthquake struck late Wednesday just south of Fukushima in the Ibaraki state. Officials said the temblor posed no danger to the plant, and that it did not cause any damage or injuries in the region.

Heavy rains prompted floods and caused road damage earlier in dozens of locations in Nagoya and several other cities, the Aichi prefectural (state) government said.

Parts of Japan's central city of Nagoya, about 170 miles (270 kilometers) west of Tokyo, were flooded near swollen rivers where rescue workers helped residents evacuate in rubber boats.

Police in nearby Gifu prefecture said a 9-year-old boy and an 84-year-old man were missing after apparently falling into swollen rivers.

More than 200 domestic flights were canceled and some bullet train services were suspended.

Toyota Motor Corp., Japan's No. 1 automaker, shut down its plants as a precaution.

Machinery maker Mitsubishi Heavy Industries told workers at its five plants to stay home, company spokesman Hideo Ikuno said.

Nissan Motor Co. spokesman Chris Keeffe said workers at its Yokohama headquarters and nearby technical facilities were being told to go home early for safety reasons, and that two plants were not operating.

A typhoon that slammed Japan earlier this month left about 90 people dead or missing.

S&P cuts Italy ratings one notch, outlook negative (Reuters)

(Reuters) ? Standard and Poor's downgraded its unsolicited ratings on Italy by one notch to A/A-1 and kept its outlook on negative, a major surprise that threatens to add to concerns of contagion in the debt-stressed euro zone.

The single currency skidded over half a cent to $1.3606 after S&P said the cut reflected its view of Italy's weakening economic growth prospects.

Italy's fragile governing coalition and policy differences within parliament will likely limit the government's ability to respond decisively to the challenging domestic and external macroeconomic environment, the agency said.

"In our opinion, the measures included in and the implementation timeline of Italy's National Reform Plan will likely do little to boost Italy's economic performance, particularly against the backdrop of tightening financial conditions and the government's fiscal austerity program," said S&P.

The move from S&P came as a surprise as the market had thought Moody's was more likely to downgrade Italy first. Moody's last week said it would take another month to decide on its action.

The downgrade came as Greece struggles to meet demands from lenders for yet more austerity measures.

"It's just more of the same negative news," said Stephen Roberts, a senior economist at Nomura in Sydney.

"It only adds to the contagion risk over Greece and has encouraged the flight to safety in markets here," he added, pointing to a sharp fall in the Australian dollar on the news.

S&P 500 futures also dropped 0.7 percent and early hopes for a bounce in Asian shares on Tuesday looked to be still-born now.

European stocks had already slid on Monday, while yields on Italian and Spanish bonds rose sharply on fears of a Greek default, compounded by the failure of EU finance ministers to agree new steps to resolve Europe's debt crisis at weekend talks.

International lenders told Greece on Monday it must shrink its public sector and improve tax collection to avoid running out of money within weeks as investors spooked by political setbacks in Europe dumped risky euro zone assets.

Finance Minister Evangelos Venizelos held what Greece termed "productive and substantive" talks by telephone with senior officials of the European Union and International Monetary Fund after promising as much austerity as necessary to win a vital next instalment of aid.

Before the talks, which will resume on Tuesday evening after meetings of experts through the day, the IMF representative in Greece spelled out steps Athens must take to secure the 8 billion euro loan payment it needs to pay salaries and pensions next month.

Large Mexican newspapers drop most sex ads (AP)

MEXICO CITY ? Two of Mexico's largest newspaper groups say they have stopped running most of the sex ads that once blanketed the back pages of popular tabloids.

The newspaper El Universal says it has banned "ads that could be used by traffickers of people." It is part of an effort to combat what experts call a huge problem of exploitation of women and children in Mexico.

The newspaper Reforma also says it has canceled sex ads.

Both paper's tabloid editions on Tuesday continued to run ads for what appear to sexually oriented phone chat services, but escort-style ads had disappeared.

Neither paper specified what guidelines they were applying in the ban, and some other papers continued to run escort ads.

Clinton mum on averting UN showdown over Palestine (AP)

SAN FRANCISCO ? Secretary of State Hillary Rodham Clinton would not predict success Thursday for last-ditch talks with Israel and the Palestinians to avert what other diplomats have called a disastrous Palestinian plan to seek statehood recognition at the United Nations.

She warned the Palestinians that their effort is shortsighted and cannot succeed.

Two senior U.S. diplomats are making their second visit in a week to Israel and the West Bank. Other talks are going on in Europe as the U.S. and its allies try to head off the Palestinian plan for a showdown at next week's U.N. General Assembly session.

The U.S. supports a Palestinian state but says it must be created through negotiation, not by U.N. action.

"I cannot give you the odds on how successful our entire effort will be," Clinton told reporters following joint security meetings with Australian diplomatic and defense officials.

She said she would give no specifics on the latest talks, calling them sensitive, but she issued a warning to the Palestinians.

The difficult issues that have held up creation of a Palestinian state, including the borders and a future security relationship with Israel, "will not be resolved if some other route is taken at the United Nations."

U.S. officials in Washington said one proposed formula would include a statement from the international Quartet of Mideast mediators ? the U.S., EU, Russia and U.N. ? endorsing the 1967 boundaries that separated Israel and the Palestinians, which President Barack Obama has said should be the basis of an agreement. This, they said, might be accompanied by some sort of conciliatory speech by Israeli Prime Minister Benjamin Netanyahu to the U.N. next week.

In a direct challenge to the United States, the Palestinians said Thursday they will ask the U.N. Security Council next week to accept them as a full member of the United Nations, even though Washington has promised to veto the measure.

In Ramallah, West Bank, Palestinian Foreign Minister Riad Malki told foreign journalists Thursday that the Palestinians were not looking for a fight. But he said the American stance puts the U.S. in a "confrontational position" with the rest of the world, and suggested American credibility could be at stake.

In Washington, White House spokesman Jay Carney emphasized that negotiation with Israel was the only viable path to Palestinian statehood.

"The Palestinians will not and cannot achieve statehood through a declaration at the United Nations. It is a distraction, and in fact, it's counterproductive," he said, adding that "the only way to resolve the issues between the Palestinians and the Israelis, and to ultimately create a Palestinian state, is through direct negotiations."

The Palestinians seek an independent state in the West Bank and Gaza Strip, with east Jerusalem as their capital. Israel captured the territories in the 1967 Mideast war.

Netanyahu rejects a complete pullout from the West Bank and says Israel must retain east Jerusalem, which it considers an inseparable part of its capital. Israel withdrew from Gaza in 2005.

The Palestinians say they are turning to the U.N. in frustration after years of failed peace talks. While a U.N. vote will not change the situation on the ground, the Palestinians believe it will improve their position in future talks. In particular, they say Israel must accept the 1967 borders as the basis of a future agreement.

While largely symbolic, the Palestinians are guaranteed a victory in the chamber, which is dominated by developing nations sympathetic to their cause. The Palestinians say some 130 nations have already pledged to support them, and Malki predicted up to 170 countries would vote in favor.

Breast, cervix cancer growing threats to poor: study (Reuters)

LONDON (Reuters) ? The number of cases and deaths from breast and cervical cancer is rising in most countries across the world, especially in poorer nations where more women are dying at younger ages, according to a global study of the diseases.

Researchers from the Institute for Health Metrics and Evaluation (IHME) at the University of Washington found breast cancer cases more than doubled around the world in just three decades, from 641,000 cases in 1980 to 1.6 million cases in 2010 -- a pace that far exceeds global population growth.

During the same period, deaths from breast cancer rose from 250,000 a year to 425,000 a year -- a much slower increase, suggesting that screening and treatment programs now common in wealthier countries are having a positive impact.

The number of cervical cancer cases rose from 378,000 cases in 1980 to 454,000 in 2010, and deaths from cervical cancer rose at almost the same pace as cases, according to the study published in The Lancet medical journal on Thursday.

"Women in high-income countries like the United States and the United Kingdom are benefiting from early cancer screenings, drug therapies, and vaccines," said Rafael Lozano, a professor of global health at IHME who worked on the study. "We are seeing the burden of breast and cervical cancer shifting to low-income countries in Africa and Asia."

He said this was one of the early signs of the emerging threat of so-called non-communicable or chronic diseases, in developing countries. "Everyone has been talking about that threat. Now the trend is clear," he said.

The study found that since 1980, new cases and deaths from cervical cancer have increased mainly in south and east Asia, Latin America, and Africa, but have declined substantially in high-income countries, particularly in countries where widespread screening is available.

However, the disease still killed 200,000 women around the world in 2010, of whom 46,000 were from developing countries and were of reproductive age.

The researchers said the findings added urgency to calls from public health experts to world leaders to make cancer screening, treatment, and education a priority in poor nations.

The United Nations is holding a high-level meeting dedicated to chronic diseases such as cancer, heart disease, lung disease and diabetes in New York on September 19-20.

"If more women are developing breast and cervical cancer during their reproductive years, this adds more pressure on families and societies already suffering from high rates of infectious disease and child mortality," said Mohammad Forouzanfar of the IHME, who led the study.

Breast and cervical screening programs have been in place in many rich countries for several decades, designed to catch cancers early and maximize the potential for effective treatment.

Many effective breast cancer treatments are also available to women in the rich world. More recently, national immunization programs using new vaccines from drugmakers Merck and GlaxoSmithKline have been launched to protect girls from the human papillomavirus (HPV) that causes almost all cervical cancers.

Partly as a result of these rich-world benefits, the study found that while in 1980, 65 percent of all breast cancer cases were in developed countries, by 2010, that share had shrunk to less than half. Some poorer countries saw a rise in breast cancer cases of more than 7.5 percent a year over the 30 year period studied -- more than twice the global rate.

The risk of cervical cancer is much higher in developing countries than in developed ones. Overall, 76 percent of new cervical cancer cases are in developing regions. Sub-Saharan Africa has 22 percent of all cervical cancer cases worldwide.
